چکیده
Jackson et al describe a complex timing cycle interaction in a patient with a dual-chamber implantable cardioverter– defibrillator. The intracardiac electrograms provided along with their description of an algorithmic approach to differential diagnosis is ripe with teaching points typical of the bedside rounds of yesteryear. Complex cases require not only meticulous logical analysis of the available data but also detailed knowledge of the background information necessary to apply that reasoning. In this instance, the authors used the morphology of the intracardiac electrograms, the relative timing between ventricular and atrial activation, identifying the initial chamber for the change in the rhythm, along with the background knowledge of specific device timing windows and expected responses, and functional knowledge of the SecureSense lead noise detection algorithm.

Appropriateness of Preoperative Antimicrobial Therapy Does Not Impact Operative or 5-Year Mortality for Patients with Infective Endocarditis Requiring Surgical Therapy
Results A total of 270 patients (190 men; mean age 46.2 years) met inclusion criteria. Native valves were infected in 219 (81%) patients. Appropriate preoperative antibiotics were administered to 177 (66%) patients. Multi-organism, Enterococcus, and fungal infections were more common in the inappropriately treated group. Recurrent IE was also more common in the inappropriately treated group. St...

We examined the characteristics of patients occupying elderly-mentally-ill acute and assessment beds in all mental health units within North and South Thames Regions; the proportion of patients defined as no longer requiring acute care (inappropriately located); and the unmet need for alternative service provision these patients required. Of the 1510 patients surveyed, 24.4% (n = 368) were defi...
متن کاملPrescription medication use among an aboriginal population accessing addiction treatment.
OBJECTIVES Inappropriate prescription medication use can have significant consequences. Although it is suspected that Aboriginal populations within Canada have high rates of inappropriate use, published information is lacking. To better understand this issue, we studied an Aboriginal population seeking addiction treatment. METHODS We surveyed Aboriginal clients who accessed addiction treatmen...
متن کاملEcological niche modeling in Maxent: the importance of model complexity and the performance of model selection criteria.
Maxent, one of the most commonly used methods for inferring species distributions and environmental tolerances from occurrence data, allows users to fit models of arbitrary complexity. Model complexity is typically constrained via a process known as L1 regularization, but at present little guidance is available for setting the appropriate level of regularization, and the effects of inappropriat...
متن کاملProphylaxis in elective colorectal surgery: the cost of ignoring the evidence.
INTRODUCTION Three strategies are used to prevent complications in colorectal surgery: heparin and antibiotics given perioperatively and abdominal drains placed intraoperatively. To investigate the appropriate and inappropriate use of these prophylactic techniques and to assess the costs associated with their inappropriate use, we studied patients who underwent elective colorectal procedures. ...
متن کاملImpact of placental weight and fetal/placental weight ratio Z score on fetal growth and the perinatal outcome
Objective: To classify the infants into 9 blocks based on the deviation of both placental weight (PW) and fetal/placental weight ratio (F/P) Z score and compared the incident rate of perinatal death in each of the small for date (SFD) vs. appropriate for date (AFD) vs. heavy for date (HFD) groups. Methods: The study population consisted of 93,034 placentas/infants from women who vaginally deliv...
